• In order to qualify for a gift you must reside in Dallas/Fort Worth Metro area and your child (0-17 years old) must have Sickle Cell. 

 

• You must complete intake form before you submit wish list. 

 

• Gift value NOT to exceed $25 each for the sickle cell warrior

 

• Go to Amazon.com and create your WISHLIST. After you have completed your Amazon Wishlist, Copy and paste the link into a new email and in subject line put: CHRISTMAS
